Safety Incident and Near-miss Reporting

A safety incident is an unplanned event that results in human injury, environmental or equipment damage, or interruption to normal operation.

near miss or close call is an unplanned event that has or has the potential to cause but does not actually result in human injury, environmental or equipment damage, or interruption to normal operation.

  • Call 911 as soon as possible for emergency medical assistance, fires, release of hazardous materials, or significant property damage.
  • For all other accidents involving University Employees, refer them to UM Hospital, 1500 E. Medical Center Drive, 998-8788
  • For all other accidents involving students, refer them to Health Services, 207 Fletcher, 763-4511
  • Accidents occurring after hours should be referred to the UMHS Emergency Room
  • File an incident or near miss report with EHS as soon as 
  • File appropriate EHS form as soon as possible following resolution of the emergency. see: Incident and Near Miss Report Form
  • File any additional reports required in the event of personal injury, fire, environmental spill, radiation leak, etc. see http://ehs.umich.edu/research-clinical/reporting-incidents/
  • You and your Department Safety Chair should receive a request for more information within 24 hours to help guide you through your incident investigation. CoE Safety Incident Report Form. This form may be used to update your report as new information becomes available.
  • The department safety committees should review all reports and notify the college safety committee when their review is complete.
