Additional reports may be required in the event of personal injuries, the release of hazardous materials, fires, etc. See EHS
You should receive a request for additional information that will help guide you and your Department's Safety Committee through your investigation and implementation of any remedial measures.
Contact the chair of your department safety committee HERE or the college safety committee HERE if you do not receive a request for additional information.
